Abstract For 3 years, a 3 to 4 weeks training programme has been offered by the Department of Dermatology, University of Erlangen-Nuernberg to inpatients with severe atopic eczema (AE). Favourable effects of this physical exercise on the clinical remittance of AE as well as high acceptance rate given by the participants prompted us to start a prospective study with AE-patients either with or without clinically controled sports activities.
 
In addition to otherwise similar dermatotherapy we compared the clinical courses, the results of different cutaneous neurovegetative function tests and the follow-up of both groups. Only patients without systemic or topical administration of corticosteroids were included into the study. All the patients and controls were tested with regard to particular neurovascular skin reactivity including examinations of thermoregulation, dermographism, and cutaneous microcirculation before and after defined stimuli. These investigations were done at onset and end of the patients' hospitalization.
 
Those patients who underwent additional sports therapy showed almost normalized thermoregulatory response to warmth as well as improved hyperaemia to local heating, whereas the nonparticipants failed to do. Our findings lend support to the hypothesis that the origin and severity of AE is influenced by intrinsic dysregulations of the cutaneous neurovascular system that can be improved by regular physical exercise.
